Linking grammar in real life

I'm having a hard time using grammar patterns together when I'm out. I learn them fine in my book. But when I try to explain my job or give someone directions, my brain just stops. I can't seem to link the pieces together into a real sentence. How do you guys practice this during your day? I feel like I'm just memorizing blocks that dont really fit yet.
