Chaining grammar patterns in real speech

I'm struggling with linking multiple grammar points together in a single flow. First I think of the conditional setup. Next I try to attach the main clause action. Then I realize the register is totally off. It feels so disjointed when I try to speak. How do you guys practice chaining clauses naturally without pausing to analyze the rule every single time?
